6400.82(f)In the bathroom located next to Individual #3's bedroom, the toilet paper was off the roll and placed on the back of the tank which is unsanitary. There was no soap available in the bathroom for use. Staff indicated that the individual's behavioral concerns caused them to put the items away and provide them only when needed for use. However, a review of the individual's ISP did not reveal any reasons for these items not to be readily available for use in the bathroom as per regulatory standards.Each bathroom and toilet area that is used shall have a sink, wall mirror, soap, toilet paper, individual clean paper or cloth towels and trash receptacle. The individual living in the home moved the toilet paper while using the bathroom. Toilet paper was returned to the holder at the time of the inspection.
