When/if inconsistencies are found, the LVS alerts the Program Supervisor, who in turn requests in writing to the Supports Coordinator requesting the individual's ISP be updated/revised accordingly. A team meeting will be held if further discussion on revising the individual's needs is necessary. All documentation will be filed in the individual's record. By 3/25/26, Program Supervisor will re-educate staff on the regulatory requirement.
Additionally, an Interest Survey is completed with individuals quarterly, along with a 3rd Quarter assessment. These tools will be used to identify, recommend, and document specific areas of vocational training or placement and competitive community-integrated employment. |

The signed documents will be filed in the consumer's individual record. Program Supervisor will train Lead Vocational Specialists by 3/25/26 on the regulatory requirements since this role will be responsible to obtain the signatures on the above documents. |
