a. (Lifesharing Provider)
b. Carpets will be cleaned.
c. -The Lifesharing Provider will get the carpets cleaned in the entire home within 30 days of the citation.
- (Associate Director) will go and assess the home within 30 days to make sure the correction was made.
-If the correction is not made the Lifesharing participant will be put into substitute care until corrections are made and the home is in compliance.
2. The Lifesharing team will use the assessment tool each month to assess the home during monthly monitors.
Steps
-Lifesharing staff will be trained on the Home Inspection tool
-Lifesharing Providers will be trained on the Home Inspection tool
-Lifesharing specialist will use tool during monthly monitors for both licensed and unlicensed homes
-Lifesharing homes that are not in compliance will be given 30 days to complete corrections.
-If corrections are not complete, the consumers will be placed in substitute care until the home is in compliance. |
