6400.167(a)(3)Individual #1 is prescribed Fiberlax, 625 mg. tablets, to be administered 4 tablets (2500 mg. total dosage) twice daily at 8 AM and 8 PM for constipation. The medication is packaged in blister packs with 2 tablets packaged together in each blister cell; staff pop out two blister cells at each medication administration to give the prescribed dose of 4 tablets. At the time of the inspection, the licensing representative discovered that on 8/04/2025 at 8 AM, Staff #1 administered the contents of only one blister cell, or two tablets, which was half of the prescribed dosage of the medication.Medication errors include the following: Administration of the wrong dose of medication.Agency nurse retrained the staff and an EIM was entered for the med error. 08/09/2025 Implemented
