A second door led into the bedroom from an attached bathroom, which could be accessed from the hallway via an unlocked door. The door between the bedroom and the attached bathroom was equipped with a privacy lock with the locking side on the interior of the bathroom; in other words, in addition to being a non-compliant locking mechanism, the lock could only be engaged or disengaged from the bathroom side of the door. Upon entering the door at the top of the basement steps there was shelving. On this shelving there was several poisonous substances which included Lysol toilet bowl cleaner, Lysol disinfecting spray, murphy's oil soap, antibacterial wipes for the counter tops, along with other household cleaners. These items should be locked in this home.Poisonous materials shall be kept locked or made inaccessible to individuals. At the time of the inspection there was a great value spray bottle with a green liquid inside of the bottle stored under the kitchen sink. 6400.64(a)There was a non-working toilet in the basement of this home. The area which the toilet was in had a very strong foul odor in this area. The agency reports that the toilet is not in use and no one uses the basement toilet. The area did not appear to be clean and sanitary.Clean and sanitary conditions shall be maintained in the home. Prior to the end of the inspection day, 11/2/23, The ReDCo Group Office Manager scheduled an appointment with Essig Plumbing for evaluation of the toilet that was deemed to be non-functional. On 11/3/23, Essig plumbing arrived and was able to ensure that there were no lingering problems or long-term challenges, the toilet was functional and needed to be used more frequently. 12/01/2023 Implemented
6400.66Rooms, hallways, interior stairways, outside steps, outside doorways, porches, ramps and fire escapes shall be lighted to assure safety and to avoid accidents. The doorway which exits to the side of the garage did not have an exterior light. There was a separate room off of the main level of the basement. This room lighting was not sufficient as the ceiling light was burned out. The light should be replaced to assure safety and avoid accidents.Rooms, hallways, interior stairways, outside steps, outside doorways, porches, ramps and fire escapes shall be lighted to assure safety and to avoid accidents. On 11/3/23, this provider¿s program specialist purchased a battery-operated light to be mounted at the garage exit of the group home. On 11/6/23, this light was mounted in order to ensure appropriate levels of lighting at the exit in question. Prior to the end of day on 11/2/23, the group home supervisor attempted to fix the lack of lighting by replacing old light bulbs in the basement with new ones. These attempts were unsuccessful, so the group home supervisor and program specialist entered a maintenance request work order to fix the potential electrical issues on 11/3/23. 12/01/2023 Implemented

This electrical outlet was not covered with the electrical outlet cover. Upon opening the container there was what appeared to be sugar in this container. Prior to the completion of the inspection on 11/2/23, the group home supervisor immediately discarded the container in the kitchen trash can. 6400.32(h)An individual has the right to privacy. At the time of inspection there was a hamper bin the basement of the home. Inside the hamper was empty blister packs of medication that was prescribed to Individual #7. The blister pack had personal information on it which was in an area that was accessible to anyone in the basement area.An individual has the right to privacy of person and possessions.Prior to the end of day on 11/2/23, the group home supervisor discarded all old medication blister packs appropriately by shredding the information that contained PHI, placing the remaining parts of the blister packs in the trash. Additionally, the group home supervisor did a walk-through of the remaining areas of the home to ensure that any and all PHI and client records were secured safely and per policy and regulation. 12/01/2023 Implemented
