The 8am dose was missing from the blister pack for 6/25/25 but was not initialed on the MAR as administered. | A medication record shall be kept, including the following for each individual for whom a prescription medication is administered: Name and initials of the person administering the medication. | 1. Staff who caused documentation error were present during the inspection period and initialed the MAR, indicating they administered the medication as prescribed. [ATTACHMENT 5].
2. The Program Coordinator or Program Director will complete a file review on all other individuals by 7/10/2024 and document compliance with 55 PA Code Chapter 6400.166(a)(13) on the Modified Licensing Inspection Instrument Score Sheet.
