The ladder dangled when pulled down as the length of it was too short. A makeshift platform was available to slide onto a basement step which is still potentially dangerous. Immediate Correction of Identified Violation: On 4/22/2026, the Property Manager contacted a licensed contractor to relocate the attic access to a compliant, accessible area. A site visit for the quote was completed on 4/23/2026. The relocation of the access was completed on 4/30/2026 to ensure full compliance with accessibility requirements. Evidence: Attachment 1 (Scheduling, Contractor work order, completed project notification, & photos before and after).
2. Systemic Review of All Locations: By 5/4/2026, the Property Manager/Program Director will conduct a comprehensive audit of all attic access points across all agency sites using a modified licensing inspection instrument. This audit will specifically assess compliance with 55 PA Code § 6400.111(a). Any site found to be noncompliant will have a site-specific corrective action plan developed within 15 days of the audit.
