This exceeds the annual requirement.The physical examination shall include: A gynecological examination including a breast examination and a Pap test for women 18 years of age or older, unless there is documentation from a licensed physician recommending no or less frequent gynecological examinations. Individual #1 moved into the Arc's residential program on 11/9/2022 from a provider in Allegheny county and had to have all medical providers transferred to the local area at the guardian's request. The first available appointment was when the appointment was conducted on 1/23/2023. An attempt was made to acquire medical documentation stating this, but the medical provider said such documentation must be requested when the appointment is scheduled and not after the fact. The next annual appointment is scheduled to avoid further issue and is slated to occur on 1/25/2024. 01/23/2023 Implemented
