| 6500.152(c) | Individual #1's Individual Support Plan, last updated on 2/12/25, was not revised to reflect their current needs as based on their current assessment, completed on 7/19/24, in the following health and safety skill domains: regarding the self-administration of medications, Individual #1's Individual Support Plan stated that [Individual #1] is "able to self-medicate, but would require support from others if a PRN was ever needed." However, Individual #1's assessment explained that that they can self-administer medication after initial guidance from the caregiver but requires total assistance with understanding the purpose of their medication; and regarding water safety, Individual #1's Individual Support Plan provided that Individual #1 can regulate their own water temperature and that they enjoy swimming, but require visual supervision around large bodies of water, leaving their actual swimming abilities unaddressed.
