6500.155(b) | Individual #1 ISP identifies an outcome to improve coping skills. There was no documentation of supports or services to indicate that a plan was develop to assist the individual to improve coping skills. | The ISP shall be implemented as written. | Individual #1 had an outcome in their ISP related to coping skills. However, the ISP did not specify that the coping skills was an outcome for the Day Program at PDDC. The Associated Director for Family Living contact the Behavioral Therapist to update the BSP to reflect who was responsible for tracking and documenting the Outcome. The BSP was updated to state PDDC was responsible for this is outcome. This was corrected on 12/31/12. The Associate Director then contacted the Supports Coordinator to update the ISP to reflect the change in the BSP. On 1/24/13, the SC did change the ISP and the outcome goal is associated with PDDC. For future compliance, the Director and Associate Director will be routinely reviewing the ISP for accuracy once they are approved. They will also be conducting scheduled reviews of the Program Files and documentation to ensure all outcomes listed in the ISP are being implemented. The Program Specialist will also be trained to review and document the status of outcomes listed in the ISP. The updated BSP and outcome section of the ISP will be sent as documenation. |
