| 6400.18(c) | Individual #1 sustained a distal radius fracture of the left wrist on 7/29/16 requiring treatment from an Orthopedist and the left wrist being casted. Incident #8205080 involving injuries requiring treatment beyond first aid was entered into the Enterprise Incident Management system on 8/8/16. | The home shall orally notify the county mental retardation program of the county in which the home is located, the funding agency and the appropriate regional office of mental retardation, within 24 hours after abuse or suspected abuse of an individual or an incident requiring the services of a fire department or law enforcement agency occurs.
| 1. The staff Person that entered the incident late(who was new to entering on HCSIS) was retrained on 8/8/2016 by Debra Andreas, Executive Director, that all incidents must be entered in EIM with 24 hours of occurrence. Incidents involving death must be called in to the county of record and the region. Abuse and Neglect issues must be called in to the APS system immediately upon identification and reported in EIM within 24hours. SC, AE are notified orally of such occurrences.
2. All incidents involving consumer health issues, mental health issues or that have to be reported to EIM will be called in to the agency on call system which is monitored by the Administrator of Program Support who is responsible for Incident Management. This will be done regardless of who enters the incident into EIM and even when the agency on call system is not operating (ie. During 8:30 am to 4:30 pm).
3. The Administrator of Program Support or the Risk Management/QM Assistant will check to make sure the incident is entered within the 24 hours and any phone calls required are made.
4. The Policy and Procedure for Reporting Incidents was updated by Debra Andreas on 8/8/2016.
5. All staff are trained in regulations regarding Incident Management at time of hiring and annually. The change to call On Call during work hours is being covered during House meetings over the next month as well through and email dated 10/6/2016 to all staff. [Incidents shall be reviewed in accordance with the agency's written policy and procedures on prevention, reporting, investigation and management of unusual incident to ensure timely reporting and the aforementioned updates are being implemented.
