6500.101 | At 2:30 PM on 2/25/25, the interior kitchen door leading to the basement was equipped with a key lock facing the basement side. The basement's exterior door to the outside can only be accessed through Family Member #2's bedroom door. However, Family Member #2's bedroom door was equipped with a key lock facing the basement side and a sliding-latch lock on the interior side. Therefore, an entrapment area existed in the basement. Additionally, at 2:35 PM, Life Sharing Provider #1 revealed in an interview that the basement exterior door that is only accessible by entering through Family Member #2's bedroom is often used as an exit route during fire drills. In addition to a standard door lock assembly and dead bolt, this basement exterior door is equipped with a clamping-latch lock, creating a blocked egress. [Repeated Violation-3/9/23 et al & 2/27/24 et al] | Stairways, halls, doorways and exits from rooms and from the home shall be unobstructed. | The kitchen and Family Member #2's bedroom door knobs/locks have been removed and will be replaced with knobs that do not require a key lock. The clamping-latch lock on the basement exterior door has been removed. Interior and exterior exit doors at all sites will be examined / evaluated by 4/30/25 using an updated monthly monitoring form. Any doorknobs with key locks or any other lock which might block egress or create an entrapment area will be removed and/or replaced immediately. Any extra locks or chains, etc. on exit doors will be removed. All Caregivers will be informed of regulations regarding door locks and egresses via the annual licensing letter by 3/31/25. Documentation of the monthly checks will be maintained in a shared teams folder with program specialists and managers of the life sharing program. |
