| 6400.141(c)(4) | Individual #2's annual physical dated 7/11/24 recommended a hearing evaluation by a specialist and it was not completed. | The physical examination shall include: Vision and hearing screening for individuals 18 years of age or older, as recommended by the physician. | 1. The Annual Physical Form was updated to include notification to the PCP if the individual is seen by a specialist for hearing/vision screening. 2. The Program Coordinator or Program Director will complete a file review on all other individuals by 7/10/2024 and document compliance with 55 PA Code Chapter 6400.141(c)(4) on the Modified Licensing Inspection Instrument Score Sheet.
