6500.72(b)Doors shall be in good repair. At the time of the inspection, I went out the kitchen door, which lead to the back patio area. The screen door handle had broken making me unable to get back into the home. The staff on the inside of the door attempted to open the door from inside, however that handle also broke. After several minutes we were able to open the screen door. The handle on the screen door is broke and needs to be repaired.Screens, windows and doors shall be in good repair.The broken handle on the screen door was repaired immediately following the inspection to ensure the door is fully functional and secure. It appears the family attempted to repair the handrail by the look of the zip tie that was wrapped about the handrail connecting that to the porch post.An interior stairway exceeding two steps that is accessible to individuals, ramp and outside steps exceeding two steps, shall have a well-secured handrail.The Lifesharing provider arranged for the handrail to be properly repaired and re-secured following the inspection. On the Sept 2025 medication administration record there were several medications including anastrozole, desitin paste, hydrocortisone cream, omeprazole, Otezla, oyster shell, pravastatin, propranol, and secura dimethicone cream all which noted there was documentation errors though out the month of Sept. The comment in the section reflects the med was given on time, however there was no futher details into the reason why it was not documented at the time of the administration. The documentation error occurred over 30 times through out the month of Sept on various medications and dates.The information in subsection (a)(12) and (13) shall be recorded in the medication record at the time the medication is administered.The Lifesharing provider responsible for medication administration was re-trained on proper documentation procedures, emphasizing that all medications must be documented immediately after administration.
