6400.163(c) | Individual #1's psychiatric medication reviews were completed 12/6/17 then again on 3/16/18. | If a medication is prescribed to treat symptoms of a diagnosed psychiatric illness, there shall be a review with documentation by a licensed physician at least every 3 months that includes the reason for prescribing the medication, the need to continue the medication and the necessary dosage. | Medication review form will be revised to include date by which follow up appointment must be scheduled to ensure compliance with the 3 month medication review requirement. This revision will be completed by VP of Intellectual Disabilities by June 1, 2018. Supervisory staff will be trained in the use of the revised medication review form by Program Directors on or before June 15, 2018. [At least quarterly for 1 year, a designated trained staff person shall audit all individuals' psychiatric medication reviews to ensure timely competition. Documentation of audits shall be kept.
