6400.67(a)Surfaces shall be in good repair. The baseboard heat cover located next to the toilet in the bathroom adjoining Individual #1's bedroom was extremely rusted and appeared corroded. The wall located in the basement farthest from the stairs had cracks between the blocks and there was what appeared to be stains from something dripping on the wall below the cracks, indicating that water may possibly be seeping into the basement through the cracks. There was an extremely rusted medicine cabinet located above the sink in the basement bathroom.Floors, walls, ceilings and other surfaces shall be in good repair. Baseboard in the bathroom adjoining individual's #1 bedroom was resurfaced and repainted on 3/19/24. The cracks between the blocks in the basement wall was repointed with mortar on 3/20/24. 6400.110(f)The bed shaker in Individual #2's bedroom did not function when the smoke detectors were tested at the time of the inspection. A wire was discovered to be disconnected. The wire was reconnected, and the bed shaker did function when retested. If one or more individuals or staff persons are not able to hear the smoke detector or fire alarm system, all smoke detectors and fire alarms shall be equipped so that each person with a hearing impairment will be alerted in the event of a fire. The disconnected wire from the bed shaker was reconnected and the bed shaker worked properly when retested during the time of inspection on 3/6/24.
