2. Long Term ¿ Maintenance of Compliance
a. Physical form was changed to highlight the items regarding Medical Information pertinent to diagnosis and treatment in case of emergency by Program Specialist, Hilary Cogley at the time of licensure. An up to date copy has already been supplied.
b. Records Clerk, Program Specialists, Program Coordinators were trained on April 25 and Site Supervisors received training on May 3 on the new physical forms and that no item would be left blank. Except for the Records Clerk, one or more of these staff will be attending the annual physical for current consumers.
c. One of the nurses will review the physical to make sure the ALL medical/physical issues are listed on the physical and make note for the ISP is physician adds any new ones
d. The Records Clerk will review all physicals immediately upon receipt and notify the Executive Director, Deb Andreas and Administrator of Program Support, Leslie M Randolph if the form has not been completed in full.
e. Physician¿s will be contacted by the nurses to retrieve any missing information needed.
f. The Record Clerk will be reviewing all remaining physicals with the nurses and make sure that the responsible physician is contacted by the nurses for any missing or incomplete information. This project will be completed by July 5, 2016.[Documentation of all reviews shall be kept. (AS 6/8/16)] |
