At 11:58AM on 2/28/2023, there was previously used food tin can containing a multitude of cigarette butts, on the wood deck near the exit door at the rear of the home.Written smoking safety procedures shall be followed.Program Specialist, conducted a site inspection on 3/8/2023 and removed the non-fire safe container previously used for cigarette butts from the home. This is noted on her 3/8/2023 site inspection (Attachment #20). Program Specialist also noted that she took a fire safe container to the home (Attachment 21). 6400.171At 11:59AM on 2/28/2023, a carton of eggs with a "best by" date of 2/8/2023 was on the top shelf in the refrigerator in the kitchen of the home.Food shall be protected from contamination while being stored, prepared, transported and served. A staff meeting was help with all Program Specialists on 3/6/2023 to address compliance and 6400 regulations. (Attachment #1). Program Specialist, held a staff meeting to review 6400.171 with house staff and stress the importance of checking labels for best by, sell by and expired dates daily.(Attachment #22). Staff are expected to discard all food items that are past their best by, sell by, or expired dates. Program Specialist, Shelly Spanopoulos conducted a site inspection on 3/8/2023 and noted no expired foods were present in the home. (Attachment #20). 03/08/2023 Implemented

The hot water temperature at bathroom sink measured 124.5 degrees Fahrenheit at 9:08 AM on 03/04/22.Heat sources, such as hot water pipes, fixed space heaters, hot water heaters, radiators, wood and coal-burning stoves and fireplaces, exceeding 120°F that are accessible to individuals, shall be equipped with protective guards or insulation to prevent individuals from coming in contact with the heat source.
