6400.163(a) | A medication pill that resembled metaprol was lying in isolation on the bottom of Individual One's medication box while performing a medication review. It was not in its original container. | Prescription and nonprescription medications shall be kept in their original labeled containers. Prescription medications shall be labeled with a label issued by a pharmacy. | It is essential that medication be stored in original containers and labeled appropriately. Any loose medication discovered should be disposed of according to protocol with our pharmacy. On 3/24/25, medication boxes were cleaned and organized. Additionally, new medication boxes were ordered to ensure organized, neat and hygienic conditions for the medication storage. Staff were retrained on ensuring proper medication storage, and proper medication disposal (see Attachment #1). |

6400.166(a)(12) | Tylenol PRN blister pack had multiple blisters popped out, but the medication administration log for the that medication did not have any dates signed for administration of med. | A medication record shall be kept, including the following for each individual for whom a prescription medication is administered: Date and time of medication administration. | It is essential that all medications administered are documented appropriately on the MAR. An audit held on 3/24/25, discovered that the last PRN administration for Individual 1 occurred on 2/28/25, however staff failed to initial the MAR (see Attachment #2). Staff were retrained on the importance of documentation for all medication administration to include PRN¿s (see Attachment #1). |
