There were several documentation errors noted for this resident that were the result of staff failing to initial the MAR appropriately. Staff were retrained on the MAR, and the importance of ensuring all steps of medication administration are completed. Staff were also trained and counseled on how to transcribe correctly on the MAR and how to capture any noted documentation errors. Retraining was completed for all full- time staff responsible for the February MAR at the Shiloh site on 2/25/25 (See Attachment #1). 02/25/2025 Implemented
