Staff were instructed to immediately clean the bathtub using sanitary practices and infection control procedures- gloves, handwashing, proper disposal of feces into toilet. The staff cleaned and sanitized the bath tub and mat (Attachment #13). On 3/8/2023, Program Specialist had a staff meeting and provided training on CDC infection and control procedures, the importance of cleaning and appropriate housekeeping procedures as well as maintaining sanitary conditions in the home at all times. 6400.65The mechanical ventilation fan in the hallway bathroom was inoperable.Living areas, recreation areas, dining areas, individual bedrooms, kitchens and bathrooms shall be ventilated by at least one operable window or by mechanical ventilation. A repair order was placed with maintenance. On 3/5/2023, Touch-Stone Solutions hired an electrician to repair the issue (Attachment #16). 6400.72(a)There is not a screen in Individual #1's bedroom window.Windows, including windows in doors, shall be securely screened when windows or doors are open. Replacement screens were ordered on 3/7/2023 from Byler's Superior Windows. (Attachment #4). The new screen will not be available for pickup until Monday, March 13, 2023. Once picked up, Touch-Stone maintenance will install the new screen. The new screens will not be available for pickup until Monday, March 13, 2023. Once picked up, Touch-Stone maintenance will install the new screen. Touch-Stone Solutions maintenance used screen repair tape to cover holes on 3/7/2023 as an immediate fix until new screens arrive. (Attachment 7). 6400.101There is a hook and eye lock on the screen door at the back exit of the home posing an obstructed egress.Stairways, halls, doorways, passageways and exits from rooms and from the building shall be unobstructed. Touch-Stone maintenance removed the hook and eye latch from the door on 3/3/2023 (Attachment #18). The hole was filled in with caulk. 6400.214(b)Individual #1's most recent dental examination was not present at the home. The most current copies of record information required in § 6400.213(2)¿(14) shall be kept at the residential home. Program Specialist, immediately corrected this on 3/3/2023. The most recent ISP and dental examination were added to the client book that is kept at the home in the staff office. On 3/8/2023, Program Specialist conducted an on-site inspection on 3/8/2023.
