6400.110(e)At 12:22 PM on 10/3/25, all smoke detectors were operable in this three-floor home. However, the only floors that were interconnected with one another were the basement and the second floor. The first floor was not interconnected with any of the home's other floors.If the home serves four or more individuals or if the home has three or more stories including the basement and attic, there shall be at least one smoke detector on each floor interconnected and audible throughout the home or an automatic fire alarm system that is audible throughout the home. The requirement for homes with three or more stories does not apply to homes licensed in accordance with this chapter prior to November 8, 1991. Prior to opening 14 Emerson in February 2025, Laurel House Maintenance Team installed interconnected fire alarm/smoke detectors throughout the home. During the 2025 Licensing Inspection, a licensing inspector attempted to set off the interconnected alarms but did not hold the test button in long enough to set off the chain of alarms. Laurel House Maintenance went to the home and tested the interconnected alarms. (a video has been attached). Staff working at 14 Emerson will be retrained on the use of the interconnect alarms to make sure that they are functioning properly. 11/26/2025 Implemented
6400.18(a)(4)Enterprise Incident Management # 9611124 for physical abuse was discovered on 4/25/25 at 10:08 AM and reported on 4/28/25 at 11:13 AM.The home shall report the following incidents, alleged incidents and suspected incidents through the Department's information management system or on a form specified by the Department within 24 hours of discovery by a staff person: Abuse, including abuse to a individual by another client. During the reporting of this incident it was noted that there had been confusion between the reporting agency and Laurel House Inc as to which agency was to file the incident. To prevent late reporting the Incident Manager retrained Laurel House Inc Program Supervisors/Specialists on appropriate time frames for reporting incidents. Laurel House Inc also had the Training Coordinator schedule regular Incident Management Training for all DSPs working in the residential homes to ensure that they are all aware of reporting guidelines and what constitutes reportable incidents. Training sign in sheets and curriculum have been attached for review. During the course of the investigation, it was found that the bruise was from bloodwork that had been completed the previous week and not by another individual. The initials KM are of her staff who took her for the blood work. 11/06/2025 Implemented
