HR Coordinator reviewed record for Staff#2 who's record was missing the validation of PA residency. Staff#2 had been terminated as of 6/24/24 so the HR coordinator wasn't able to attain verification. HR coordinator reviewed current new hires since on-site inspection to ensure that a prospective employee who resides/resided outside of PA has had FBI criminal history check submitted to the FBI plus the PA criminal history check. HR Business Partner retrained HR Coordinator 9/12/2024, on the importance of ensure the question attesting to PA residency is completed on each "consent for background" form. 09/12/2024 Implemented
6400.77(b)There was no tape located in the first aid kit. A first aid kit shall contain antiseptic, an assortment of adhesive bandages, sterile gauze pads, a thermometer, tweezers, tape, scissors and syrup of Ipecac, if an individual 4 years of age or younger, or an individual likely to ingest poisons, is served. Group Home Supervisor purchased a new roll of tape and added it to the first aid kit on 6/26/24. The Direct Support Supervisor was trained on 8/8/24 by GH IDD Administrator on the use of the first aid kit checklist. 06/26/2024 Implemented
