The question attesting to living in the state of PA for the past 2-years at the time of hire was left blank on staff #3's application form. Due to the question being left blank, it is unclear whether the staff resided in the state for 2 years prior to the date of hire. The FBI check that was completed by a third-party hiring entity was not completed through the PDA (Department of Agin) as required.
The third-party vendor that is assisting the agency in completing background checks, is not acquiring FBI checks through the PDA (Department of Aging) as required. | If a prospective employe who will have direct contact with individuals resides outside this Commonwealth, an application for a Federal Bureau of Investigation (FBI) criminal history record check shall be submitted to the FBI in addition to the Pennsylvania criminal history record check, within 5 working days after the person's date of hire.
| HR Coordinator reviewed record for Staff#2 who's record was missing the validation of PA residency. Staff#2 had been terminated as of 6/24/24 so the HR coordinator wasn't able to attain verification. HR coordinator reviewed current new hires since on-site inspection to ensure that a prospective employee who resides/resided outside of PA has had FBI criminal history check submitted to the FBI plus the PA criminal history check. HR Business Partner retrained HR Coordinator 9/12/2024, on the importance of ensure the question attesting to PA residency is completed on each "consent for background" form. |
