6400.64(a)At 11:25 AM on 11/14/2024, the interior base and door of the oven in the kitchen of the home had baked on brownish-colored grease, chard food particles, and several spots of a white powdery substance.Clean and sanitary conditions shall be maintained in the home. The stove located at the residential site came with the home when it was purchased by Laurel House, Inc. Staff at the residential site have cleaned the home but because of its age, the discoloration and spots on the stove will not come clean. Laurel House, Inc to purchase a new stove for the home. laurel House, Inc Maintenance Team to install the new stove. Program Supervisors/Specialists will review the cleanliness and functioning of appliances during their weekly visits and note any needed corrections or maintenance attention. 6400.64(e)At 11:39 AM on 11/14/2024, an open, white garbage bag with Dunkin Donuts paper wrapping and other discarded items was hanging on the laundry room door of the home; outside of a trash receptacle.Trash receptacles over 18 inches high shall have lids. DSPs in the home removed garbage bag from the door on 11/14/24 and placed it into an appropriate trash receptacle that had a lid. Program Supervisors/Specialists will ensure that during their weekly visits all trash is placed in the appropriate trash receptacles at the home. If any new receptacles with lids are needed at the home a request will be placed for them to be ordered. DSPs will also notify Program Supervisors/Specialists prior to their visits if receptacles need to be ordered for the home. Visit forms to be submitted to licensing for review. 12/31/2024 Implemented
6400.70At 12:05 PM on 11/14/2024, the home's only telephone was inaccessible to Individual #1 and Individual #2 in the staff bedroom.A home shall have an operable, noncoin-operated telephone with an outside line that is easily accessible to individuals and staff persons. On 11/14/2024 DSP at the residential site moved the phone from the staff room to the front of the home where it is accessible to all individuals and staff at the home. The Emergency phone numbers which include: 911, poison control center, mental health crisis line, the local hospital, local police, state police, laurel house main office, and laurel house on call number, were also placed near the home. 6400.72(a)At 11:33 AM on 11/14/2024, the left window, in the living room facing the side of the home, did not have a screen. At 11:52 AM on 11/14/2024, the right window, in Individual #1's bedroom facing the front of the home, did not have a screen.Windows, including windows in doors, shall be securely screened when windows or doors are open. On 12/11/24 Laurel House, Inc Maintenance Team built new screens for the living room window and the window in Individual #1's bedroom. Laurel House, Inc will submit a photograph of the screens to licensing. Laurel House Hazard Checklist and Maintenance Form will be submitted to licensing for review. 12/31/2024 Implemented
6400.72(b)At 11:52 AM on 11/14/2024, the screen in the right window facing the rear of the home in Individual #2's bedroom was damaged with the following: two small circular holes; a jagged, linear tear measuring three inches in length; and an irregularly shaped tear measuring one inch wide by one-half inches in length. Screens, windows and doors shall be in good repair. On 12/11/2024 Laurel House, Inc Maintenance Department removed the old screen from the window screen and rescreened the frame. Laurel House, Inc will provide a photograph of the repaired screen to licensing for review along with Laurel House, Inc Visit Forms and Maintenance Request Form for review. 12/31/2024 Implemented
6400.77(b)At 12:08 PM on 11/14/2024, the home's first aid kit did not contain a thermometer. A first aid kit shall contain antiseptic, an assortment of adhesive bandages, sterile gauze pads, a thermometer, tweezers, tape, scissors and syrup of Ipecac, if an individual 4 years of age or younger, or an individual likely to ingest poisons, is served. Laurel House, Inc Nursing Coordinator ordered thermometers to be included in the First Aid kits that are located at the residential sites. Nursing Coordinator and Agency Nurses reviewed regulations regarding the contents of the first aid kits at the homes. 12/31/2024 Implemented
6400.110(e)At 12:06 PM on 11/14/2024, the smoke detectors of this three-story home were not interconnected.If the home serves four or more individuals or if the home has three or more stories including the basement and attic, there shall be at least one smoke detector on each floor interconnected and audible throughout the home or an automatic fire alarm system that is audible throughout the home. The requirement for homes with three or more stories does not apply to homes licensed in accordance with this chapter prior to November 8, 1991. On 12/9/2024 Laurel House, Inc Maintenance Department installed 10 interconnected smoke/CO2 detectors throughout the home. It should be noted that according to Fayette Co Properties, this residential site is considered a 1 story Ranch Style home with a Full Basement. Program Supervisor/Specialist and DSPs in the home were trained on working the interconnected smoke alarm system. Laurel House, Inc will provide Home Hazard/Safety Checklist and Fire Drill Forms for review to licensing. 12/31/2024 Implemented
6400.111(a)On 11/14/24, the garage/laundry room floor in between the home's split-level main floor and the game room, did not have an operable fire extinguisher with a minimum 2-A rating.There shall be at least one operable fire extinguisher with a minimum 2-A rating for each floor, including the basement and attic. On 11/14/2024 Laurel House Maintenance Department installed a new fire extinguisher from A&I Fire Extinguisher Company in Uniontown, at the residential sight. The fire extinguisher meets regulatory minimum rating requirements. Laurel House, Inc will submit Fire Drill Form to licensing for review. 12/31/2024 Implemented
6400.111(f)On 11/14/2024, the fire extinguisher in the kitchen of the home did not have date of inspection. A fire extinguisher shall be inspected and approved annually by a fire safety expert. The date of the inspection shall be on the extinguisher. On 11/14/2024 Laurel House, Inc Maintenance Department installed a new fire extinguisher from A&I Fire Extinguisher Company in Uniontown at the residential site that replaced the fire extinguisher that was missing its inspection tag. The fire extinguisher met regulatory minimum rating requirements. Laurel House, Inc will submit a photograph of the new fire extinguisher and fire extinguisher forms for review. 12/31/2024 Implemented
6400.112(c)The written fire drill records for the fire drill held on 4/28/24, 7/12/24, and 8/23/24 used the garage door as the exit route. The garage does not have a swing door.A written fire drill record shall be kept of the date, time, the amount of time it took for evacuation, the exit route used, problems encountered and whether the fire alarm or smoke detector was operative. Program Supervisors/Specialists will review the fire drills/fire safety at each of the homes to ensure that proper verbiage is used when explaining which exit is used at the residential site. DSPs at this residential site were informed that when exit for a fire drill through "the garage door" they should state that they are exiting through the sunroom door.
