| 6400.64(a) | Individual #1's bedroom has a strong urine odor. | Clean and sanitary conditions shall be maintained in the home. | The individuals' hospital bed matrress was replace as it was the contributing factor to the urine smell. Staff have been counseled to clean new mattress daily and also change sheets daily.
A monthly home inspection checklist has been developed and will be completed by Program Directors monthly and turned into CEO & COO for review to make sure any repairs needed are reported and completed promptly.
Monthly Home inspection form will be emailed to BHSL by 02/26/15 along with documentation of completed repair.

| 6400.67(a) | The front hallway bathroom has damage to the outside of the shower wall 8inches X 10inches in size above the floor.
Bathroom #2 has a broken retainer for the hand held shower head, and the metal flexible hose is rusted.
| Floors, walls, ceilings and other surfaces shall be in good repair. | The shower hose/head was replaced by maintenance on 02/13/2015 and also the the damage to the outside shower wall was repaired , spackled and painted by maintenance staff and completed on 02/24/15.
