On 3/20/2024 at 11:10am the lights at both of these exits were inoperable and there was no other lighting source to illuminate these egresses in the case of an emergency evacuation.Rooms, hallways, interior stairways, outside steps, outside doorways, porches, ramps and fire escapes shall be lighted to assure safety and to avoid accidents. Maintenance staff replaced the two lightbulbs that were inoperable during the on-site inspection on 3/20/24. 6400.72(a)On 3/20/2024 at 11:20am, the window located in the living room on the right side of the television was observed with an accordion-style screen rather than a secure screen to protect against a potential infestation. On 3/20/2024 at 11:23am, the window located in individual #1's bedroom on the left side of the bed was observed with an accordion-style screen rather than a secure screen to protect against a potential infestation. On 3/20/2024 at 11:25am, the window located in individual #1's second-floor television room on the right side of the television was observed with an accordion-style screen rather than a secure screen to protect against a potential infestation. On 3/20/2024 at 11:27am, three windows located in the attic of the home were observed with accordion-style screens rather than secure screens to protect against a potential infestation.Windows, including windows in doors, shall be securely screened when windows or doors are open. The maintenance supervisor purchased screening materials to make custom fit screens. All 6 accordion-style window screens were replaced at the home on 3/26/24 with the custom fit screens made by maintenance staff that ensure protection against possible infestations (see emailed documents #5, #6, #7, #8, #9, and #10). On 3/28/24, maintenance staff were retrained in the regulation requirements specific to this violation, as well as, their responsibilities in custom fitting screens to any new windows or doors that may be purchased in the future (see emailed document #11). 03/28/2024 Implemented
