On 10/8/2025 Laurel House Maintenance Team went to the residential site and adjusted the temperature of the hot water tank to within regulatory temperature (117 degrees). Maintenance rechecked the water temperature for the next 4 days, to ensure that the temperature stayed at 117 degrees. Documentation attached for review 10/08/2025 Implemented

Staff interviews revealed that the home's telephone is kept in the office due to Individual #1 making excessive night calls to their parents who reside in a different time zone.An individual has the right to unrestricted and private access to telecommunications.DSP at the residential site moved the phone from the staff room to the living room area of the home. The Emergency phone numbers which include: 911, poison control center, mental health crisis line, the local hospital, local police, state police, laurel house main office, and laurel house on call number, were also placed near the home.
