On 12/7/22 at 11:42AM, the light fixture outside the basement door was inoperable. There is not another source of lighing in this area. On 12/7/22 at 11:54AM, there was not source of outside lighting at door leading from the dining room to the outside of the home.Rooms, hallways, interior stairways, outside steps, outside doorways, porches, ramps and fire escapes shall be lighted to assure safety and to avoid accidents. The maintenance department fixed the two ceiling light fixtures near the furnace and hot water tank, the light fixture outside the basement door and the light source at the door leading from the dining room to the outside of the house.. 6400.101On 12/7/22 at 11:39AM, the kitchen side of the door between the kitchen and the basement was equipped with a key-locking doorknob and deadbolt obstructing egress from basement when engaged. On 12/7/22, at 11:43AM, the door from the basement to the outside of the home was equipped with two latch locks obstructing egress when engaged. On 12/7/22 at 11:43AM, the basement door leading to the outside of the home was equipped with two vertical deadbolt locks, at the top and bottom corners of the door, obstructing egress from the basement when engaged. On 12/7/22 at 11:48AM, the basement side of the door between the basement and the garage is equipped with eye-hook locking mechanism obstructing egress from the garage when engaged. There is not a man-door in the garage.Stairways, halls, doorways, passageways and exits from rooms and from the building shall be unobstructed. The maintenance department took off the locking mechanism to provide unobstructed egress from the basement to the kitchen. Picture is enclosed. The latch locks were taken off the door in the basement leading to the outside of the home and the vertical deadbolt locks were removed from the top and bottom corners of the door. Pictures are provided. on the basement side of the door between the basement and garage, the eye-hook locking mechanism was removed. Pictures are provided. 12/14/2022 Implemented
6400.110(f)Individual #1 has hearing loss and is prescribed bilateral hearing aids. On 12/7/22 at 12:12PM, the smoke detector in the basement of the home was not equipped with strobe lighting. If one or more individuals or staff persons are not able to hear the smoke detector or fire alarm system, all smoke detectors and fire alarms shall be equipped so that each person with a hearing impairment will be alerted in the event of a fire. The maintenance department installed strobe lights in every room on the first floor and basement to ensure the Individual (ES) can be alerted to safely evacuate her home.
