| 6400.81(k)(2) | The mattress and box springs for Individual #1 were on the floor. | In bedrooms, each individual shall have the following: A clean, comfortable mattress and solid foundation. | The client in question is a large man (over 400 lbs). Multiple bed frames have been purchased for him and have broken. Client has chosen to have his bed on the floor as a result. Program Specialist has spoken to the client regarding the need for an actual bed frame and together they have selected a solid pine bed frame that will hopefully stand up to his weight. In the event that this bed does not work out, the agency will be looking into having a custom built frame made to accommodate his large frame, |

| 6400.112(e) | The most recent fire drill conducted during sleeping hours was on 5/23/14. | A fire drill shall be held during sleeping hours at least every 6 months. | From this point forward, agency protocol will be that sleep time fire drills be performed on a quarterly basis to ensure that we not only meet but exceed the guidelines set by the regulations. Program Coordinator will train Program Specialists on this protocol. In addition Overnight drills will be added to the internal tracking system to better assist with ensuring compliance. |
