| 6400.181(a) | Individual #1's assessment, completed 12/18/2024, was sent to the plan team members on 12/19/2024 for the Individual's annual ISP meeting held on 8/19/2025. | Each individual shall have an initial assessment within 1 year prior to or 60 calendar days after admission to the residential home and an updated assessment annually thereafter. The initial assessment must include an assessment of adaptive behavior and level of skills completed within 6 months prior to admission to the residential home. | Assessment dates will be tracked by the Assistant Program Director on an ongoing basis to ensure that assessments are completed annually and within six months of the annual planning meeting. In the event an assessment is completed earlier than six months, the assessment will be reviewed again within six months prior to the ISP meeting to determine if any information needs to be updated. If the information is still accurate, it will be resent to all team members at least 30 days prior to the ISP meeting with notification that the assessment still remains accurate. If changes are necessary, another update will be completed and sent to all team members at least 30 days prior to the ISP meeting. A supervisor meeting was held on 10/8/25 and assessments were discussed to ensure future compliance. |
