This section was left blank. | The assessment must include the following information: Recommendations for specific areas of training, programming and services. | Program Supervisor/Specialist (PS) updated the Comprehensive Assessment of Functional Performance for the individuals to ensure that all areas were filled in and that the blank section has been completed in it's entirety. The PS then completed and mailed an updated letter informing the individuals Supports Coordinator, that the CAFP had recently been updated. The areas that were updated were noted in the letter. A copy of the letter and CAFP were placed in the individuals chart located at Laurel House Inc |
