| 6500.66 | The exit leading from the basement is not equipped with outside lighting. | Rooms, hallways, interior stairways, outside steps, outside doorways, porches, ramps and fire escapes, that are used by individuals shall be lighted to assure safety and to avoid accidents. | The Lifesharing Provider was informed by the Lifesharing Specialist that a light needed to be installed outside of the basement exit door. The light was installed on 2/20/2021 by the provider, a picture was taken to show the position of the light in reference to the outside exit, please see Attachment #1. The CH updated the monthly home checklist to include checking for lighting outside all exits. CH verified on 2/22/2021 that the light was installed and working properly at the home visit and a checklist was completed, please see Attachment #2. Both signed stating that they understand regulation 6500.66, please see Attachment #3. |

| 6500.121(c)(7) | Individual #1's most recent gynecological, pap smear and breast exam was completed on 10/30/19. | The physical examination shall include: A gynecological examination, including a breast examination and a Pap test for women 18 years of age or older, unless there is documentation from a licensed physician recommending no or less frequent gynecological examinations. | Individual #1 is scheduled for her annual exam on 3/22/2021, at this appointment she will receive education on the importance of gynecological exams, breast exams, pap test and the importance of the frequency of these processes. All options will be discussed that can help with her anxiety related to this process. Information from the appointment will be obtain and send in as Attachment# 1a once obtained. |
