6400.141(c)(8) | Individual #1 had a mammogram done on 2/7/2014. Individual #1's physician ordered mammograms be done every 2 years. An unsuccessful attempt at having a mammogram performed was done on 4/22/2016, which exceeds the 2 year recommendation by her doctor. | The physical examination shall include: A mammogram for women at least every 2 years for women 40 through 49 years of age and at least every year for women 50 years of age or older. | ¿ Documentation dated April 25, 2016, from Individual #1's, PCP deferred mammograms until she is 50 years old. This is based on US. Preventive Services Task Force (USPSTF) guidelines
¿ Unless Individual #1's becomes symptomatic and or there are concerns with her condition, her PCP will be contacted to schedule a Mammogram after August 30, 2022.
¿ Managers and support staff will use Mammogram Desensitization Plan, dated May 16, 2016, to provide additional support for Individual #1 to successfully complete the Mammogram procedure. |
