| 6500.121(c)(7) | Individual # 1's most recent breast examination was completed in 2009. | The physical examination shall include: A gynecological examination, including a breast examination and a Pap test for women 18 years of age or older, unless there is documentation from a licensed physician recommending no or less frequent gynecological examinations. | The Life Sharing Specialist supported the Provider and individual on 12/29/2014 with a follow-up visit where a breast examination was completed. The Life Sharing Specialist was able to train the provider on what questions to ask during the appointments. On January 7,2015, the Life Sharing Specialist conducted a monthly monitoring which she explained that the individual should be receiving a yearly breast exam along with a yearly mammogram. Going forward, the Life Sharing Specialist will be tracking the medical appointments to be summarized in Monthly and Quarterly notes to ensure all appointments kept and all regulations followed. |
