On 9-24-20 a discussion occurred between Ind. #1, Program Coordinator and Program Specialist. Per the conversation it was determined that Individual #1 does not prefer to have a lock on his bedroom door at this time. An addendum to the ISP was also sent out to include his preference for not having a lock on his bedroom door.
Excentia's annual Assessment form has been updated to include information regarding assessing a persons desire to lock their bedroom door as well as assessing the type of locking mechanism to be used. |
