6400.141(c)(6)Individual #1 had a Tuberculin skin test completed on 3-23-19, and the previous Tuberculin skin test was completed on 2-22-17.The physical examination shall include: Tuberculin skin testing by Mantoux method with negative results every 2 years for individuals 1 year of age or older; or, if tuberculin skin test is positive, an initial chest x-ray with results noted. The individual had a Tuberculin test completed on 3/11/2019, not 3/23/19. The program manager will schedule a tuberculin test on or before 3/11/2021. The program manager and the program coordinator were retrained on the 6400 regulation 141c6 : the physical shall include: tuberculin skin testing by Mantoux method with negative results every 2 years. Going forward, all mandatory appointments including the TB test will be entered and monitored in Therap, the electronic record system, by the program manager and reviewed by the program coordinator to ensure that the appointment is within the required 2 year time frame. 6400.101During the physical site inspection of the home on June 5, 2013, the basement area of the home had a door with a locking mechanism that requires a key for re-entry into the home.Stairways, halls, doorways, passageways and exits from rooms and from the building shall be unobstructed. The doornob to the garage at the Rosecrest house was changed to a non-locking type nob. All Mainstay homes were susequently inspected and all homes comply with the regulation. This was an oversight that appears to be an anomoly. Documentation from our facilities personell will be submitted. 07/05/2013 Implemented
