A retrain was held on 4/1/2022 for all supervisors who had areas of non-compliance during the licensing inspection as it pertains to fire drills and PA Code Chapter 6400.112(c). A full retrain of all areas of need for each fire drill (to include date, time, the amount of time it took for evacuation, the exit route used, problems encountered and whether the fire alarm or smoke detector was operative, review of the revised fire drill process as well as a review of the revised fire drill checklist ) was conducted in which these identified supervisors were retaught all regulations regarding fire safety. Attached is a copy of the sign off sheet which indicates that this retrain took place and that all site supervisors acknowledge that they are aware and will be responsible for initial review of all fire drills before submission to the Administrative office for second review. If areas of the fire drill are found during initial review, supervisor will ensure that feedback is given to the staff and another fire drill will be conducted during that shift with those same staff until the fire drill and all documentation is completed as needed and is compliant with Pa 55 Code standards. 04/01/2022 Implemented

TTSR has already purchased fire safety videos called ¿Fire Safety Training, Training For Anyone Including Individuals With Developmental Disabilities And For Persons Providing Them Services¿. These videos are distributed by Program Development Associates (www.Disability Training.com) and will be maintained at each site and will be used in the event that TTSR can not conduct our Annual Fire Safety Training face to face and in large numbers. TTSR is also looking into a company named Reliant to research what offerings they have in the event that we can not offer Fire Safety training in-person and need to seek approved fire safety trainings on-line for both staff and individuals. On 1/30/2017, Individual #1 was asked by house supervisor if he wanted a foundation to be placed under his box spring and mattress and Individual #1 adamantly stated that he did not wish to have a foundation. Individual #1 expressed his fear of being off of the ground and possibly falling out of bed. To respect Individual #1¿s wishes, choice, and rights, TTSR will apply for a waiver so that there is approval for Individual #1 not to have a foundation underneath his box spring and mattress. 6400.68(b)The hot water temperature in the first floor bathroom was 123.8 degrees fahrenheit at 9:30 am.(b) Hot water temperatures in bathtubs and showers may not exceed 120°F. On 1/10/2014, TTSR Maintenance worker, Chris Arnold arrived at the North First Street site located at 104 North First Street in Shippenville, PA. 16254. The purpose of this visit was to remedy an area of non-compliance as it pertains to the temperature of the water in the home which exceeded 120 degrees Fahrenheit on the day of the licensing site inspection. To fix the issue, Mr. Arnold went down he basement and approached the hot water tank. Mr. Arnold then proceeded to remove the covers (one on top and one on bottom of the hot water tank) and saw three settings which regulates the water temperature. Mr. Arnold turned the knob to a cooler setting and repeatedly measured the temperature of the water in the kitchen sink and 1st floor shower until the water temperature read lower than 120 degrees Fahrenheit. The thermometer was held on the water for one to two minutes and the readings from the thermometer consistently ranged from 111 degrees Fahrenheit and 114 degrees Fahrenheit. The attached photo shows a reading of 112 degrees Fahrenheit on the sink in the kitchen (shower temperature also read 112 degrees fahrenheit consistently and did not exceed 120 degrees fahrenheit). Water temperatures will be tested on a bi-monthly basis by TTSR Administrators who will conduct monthly site inspections for all TTSR sites operated. 01/10/2014 Implemented
