It is not clear if the individual has the ability to self-medicate based on the agencies administration practices.To be considered able to self-administer medications, an individual shall do all of the following: Know when the medication is to be taken. Assistance may be provided by staff persons to remind the individual of the schedule and to offer the medication at the prescribed times as specified in subsection (b).The individual has been able to successfully self-administer her medication for approximately 10 years and has utilized several different pill boxes in the past to help her maintain this skill. More recently, this individual had been having issue with the weekly pill box being used as some of the future days of the weeks¿ medications would come out when she turned it over to take out the medication for a specific day/time. At her request, she asked staff to minimize this issue by taking her medications out of the box daily. Recognizing that this is not best practice, a new pill box was obtained that allows this individual to still have access to her weekly medications while also providing her the ability to remove just one day/one time during self-administration. This new pill box was purchased and set up for usage effective 12/8/23 (Attachment #2- weekly pill box) 12/08/2023 Implemented

6400.71There was no emergency telephone number list near the phone in the kitchen.Telephone numbers of the nearest hospital, police department, fire department, ambulance and poison control center shall be on or by each telephone in the home with an outside line. Service Director ensured this issue was corrected the same day it was discovered (11/18/2020). In addition to hanging a list of emergency numbers next to the phone in the kitchen, the Service Director will ensure that emergency numbers are located on each phone, in all homes. This step will be completed no later than 1/31/2021. All other homes have been assessed as of 12/22/2020 to ensure no other areas of non-compliance. The Community Support Facilitator will complete a monthly inspection of each home to ensure ongoing compliance. Attachment 6: Photo of emergency numbers hung by the phone.
