| 6400.52(c)(3) | Direct Service Worker #1 did not complete the following training during the 1/01/2023 to 12/31/2023 annual training year: Individual Rights. | The annual training hours specified in subsections (a) and (b) must encompass the following areas: Individual rights. | The trainer reviewed the policy with the Direct Service worker #1 that 6400 regulations require that every direct care employee must complete and maintain at least 24 hours of in-service training on an annual basis. The in-service was completed on March 25, 2024. Also, the Direct Service Worker #1 was issued a discipline of written warning on 03/19/2024 for failure to complete her required training of the following subject (Clients rights and recognizing and reporting incidents) by one day during 2023 calendar year. The trainer, program specialist managers will be responsible in checking at the end of each month that all required training topics for that month are completed and accounted for for each Direct Service Worker including Direct Service Worker #1to ensure training completion. The Resident Service director will monitor this process quarterly by reviewing compliance with the trainer and program specialist managers to prevent future similar incidents. |
