| 6400.101 | On 2/20/25 at 10:11 am a bar was attached to the wall and rotated downward and rested on an attachment on the sliding glass door prohibiting the door from opening, thus blocking the egress. | Stairways, halls, doorways, passageways and exits from rooms and from the building shall be unobstructed.
| On 2/20/2025, the obstruction was lifted for the door to be able to open. The obstruction was not placed back down to block the door from being opened. Site Supervisor called the maintenance to have the apparatus removed. On 2/22/25, The apparatus was removed from the door. Staff were trained and made aware that the area cannot be blocked at any time 2/26/25, 2/27/25. |
