6400.81(a)Individual #1's bedroom is in the basement of the home. There is not a door from the basement that leads directly to the outside at grade level. An individual's bedroom may not be located in basements. Any level from which there is a standard door leading from that level directly outside to grade level is not considered a basement. The individual had a room in the upstairs of the home available to him and requested relocation to lower level of split level home that provided significantly more space. The room had been used as a staff office. We made the accommodation and moved individual based on an incorrect interpretation of compliance due to being a split level residence. In the meantime, his prior room in upstairs of home is now occupied by an emergency placement with APS involvement. We temporarily increased the capacity of the home to accomodate while a new CLA is prepared. We expect this to be completed in less than 4 weeks. This will allow a revision back to a 2-person residence and the consumer to re-establish a bedroom on the upper floor as his primary personal bedroom. However, he will continue to have the opportunity to utilize the finished lower level of home for recreation, storage, and activity. 05/01/2025 Implemented

6400.163(c)The 12/14/18 medication review for Individual #1 did not have if the current medications prescribed should or should not be continued. If a medication is prescribed to treat symptoms of a diagnosed psychiatric illness, there shall be a review with documentation by a licensed physician at least every 3 months that includes the reason for prescribing the medication, the need to continue the medication and the necessary dosage.Based on input from licensing, we have revised our medication review form to be more in line with provider community. The form requires notation of each psychiatrist prescription separately rather than attaching a MAR alone. This insures the need to continue is clearly associated with prescribed medications. We have also made relevant sections more prominent on the form for oversight. The updated form is attached as number 6; we do not have an example used since the update of the form and distribution to programs on 4/19/2019. A review completed since licensing will be sent as an addendum. 04/19/2019 Implemented
