| 6400.141(c)(4) | Individual #1, date of admission 9/16/2024, had an initial vision screening conducted 5/29/2025 which indicated blindness, and not prior. | The physical examination shall include: Vision and hearing screening for individuals 18 years of age or older, as recommended by the physician. | The family of this particular individual is aware of health requirements and verbally said she will try and do a better job at making sure appointments, including vision are timely. She explained that she is getting older and would like to continue until she can no longer do so. The family feels very comfortable and loves Emmaus Community of Pittsburgh. The RA reminded this particular family member that Emmaus staff and RA are available to assist with such appointments as well. The Mother noted that there have not been changes in her vision, she can not see well peripherally however. It does not impact her daily activities however. |

Especially her loved one's mammogram. The Mother explained that she is getting older and would like to continue making and following through with her appointments until she can no longer do so. The family feels very comfortable with her healthcare and loves Emmaus Community of Pittsburgh. The RA reminded the Mother that Emmaus staff and RA are available to assist with such appointments as well. The Mother said her wish is that she passes away before her daughter. Emmaus would take over the individual's healthcare at that time. |
