This soap which was equate berry white tea scent stated contact poison control. | Poisonous materials shall be stored in their original, labeled containers. | The homes are the only two homes with soap dispensers. The home's soap was checked and had non-toxic soap. Both supervisors of the homes were emailed by the Program Manager on 12/15/25 to ensure they understand the only types of soap that can be purchased. It was recommended that they stick to using the Softsoap brand. The same soap will be purchased with no changes in order to ensure soap is non-toxic. Attachment #6 is a picture of the soaps that are currently in the home and will remain in the home. |

| 6400.67(b) | At the time of the inspection, their was a golf ball size amount of lint in the dryer. | Floors, walls, ceilings and other surfaces shall be free of hazards. | On 12/14/2025, information was sent to all supervisors and staff regarding the proper time/procedure for removing lint from the dryer (when removing laundry, not when putting clothes into the dryer). See attachment #7. All dryers have been checked, randomly, by program staff between the date of inspection and 1.9.26. This has not been found to be an issue during this time period in any other location. The staff person who had taken the clothes from the dryer, but did not remove the lint, had this information reviewed with her specifically on 12.22.25/. She signed off on the information - see attachment #8. |
