6400.32(d)On 4/22/2026 at 11:40AM, an eight inches by eleven inches typed "house rules" sign was publicly posted in the hallway on the first floor of the home, disclosing personal information. The sign read "House Rules for Health Management and Independent Living Plans. The kitchen will be open at designated times, and staff will accompany the resident in the kitchen at all times. Menus will be planned in accordance with the individual's medical needs, as directed by a physician. The resident will not ask for others to share their food with them, at home, or in the community. Staff cannot share their food with the resident. Food cannot be delivered to the home. Cash and debit cards are secured in the home, and staff will provide them to the resident for necessary items or planned community outings, and the resident is responsible for returning receipts. If money will be used for food in the community, calorie counts will be planned beforehand with assistance from staff. Personal care items will be distributed by staff daily as needed. Quiet hours are observed in the residence from 11pm to 7am in order to encourage good sleeping patterns and promote everyday wellness. Internet will be accessed with staff support, and the phone shall be used for social and emergency purposes."An individual shall be treated with dignity and respect.The sign was immediately removed at the time of inspection. 05/04/2026 Implemented

Agency policy indicates that individuals may exercise their right to lock their bedroom and access their bedroom with a key, a keypad code, an access card or other entry mechanism as they choose. The individual had previously been provided with a key to their bedroom to exercise their right to lock their room as they chose to do so. Bedroom lock has been changed to digital keypads, and the individuals may use a key, or a keypad code of their choosing to lock and access their bedroom as they choose. Staff are being trained on the rights of the individuals to lock their bedroom doors, the new locking mechanisms and how to access in an emergency.
