6400.112(a)An announced fire drill was not held in August 2023. An unannounced fire drill shall be held at least once a month. All fire drills are compliant at this time. Fire drills shall be conducted monthly by the 25th of each month. If a fire drill is not conducted by the 25th of the month, the House Manager will go to the home to ensure that staff complete the drill that day before the end of the month. Fire drills are to be completed by RPW staff within the home. MCAR has RPW shifts of 1, 2 and, 3 staff. Fire Drills are assigned to a different shift each month and given a time frame to complete with a deadline of the 25th of each month. This assignment is emailed to the group homes at the beginning of each week by the Administrator of Residential. Fire Drill forms are completed electronically and once finalized a completed copy is then emailed to the residential management department. House Managers also complete a weekly checklist that must be submitted to their Residential Program Specialist each week. This checklist outlines if a fire drill was completed for the month and requires House Manager to check logs within the home for proper documentation. Black logs are kept within the homes and hold fire drill record logs that reflects each fire drill completed throughout the year. 08/23/2024 Implemented

The bottom of the cabinet under the kitchen sink has been repaired. All staff will be retrained on MCAR¿s Policy and Procedure for writing Work orders at the August 2015 in-services 8/7/2015, 8/12/2015, and 8/19/2015. This training will be conducted by either Tina Nicastro or Nicholas Gantz. A memo was sent on 6/29/2015 via email and paper copy to remind staff of the importance of writing work orders. Program specialists approve all work orders sent to maintenance. The agency shall complete a self-assessment of each home the agency operates serving eight or fewer individuals, within 3 to 6 months prior to the expiration date of the agency¿s certificate of compliance, to measure and record compliance with this chapter. The Agency will now start 6400's self-assessments the first week of November and self assessment will be completed by the last week in December of every year after. To ensure that self-assessment's are completed during that timeframe Microsoft Outlook calendar now reflect start and end dates of self-assessments and will be initialed by supervisor of completion during the allotted timeframe.
