The front sidewalk and the end of the driveway of the home were covered with snow and ice at the time of the inspection. The patio area and sidewalk at the rear exit of the home were covered with snow and ice at the time of the inspection. Outside walkways shall be free from ice, snow, obstructions and other hazards. Provider agency staff cleared all walkways and driveways around the home from snow and ice. Provider agency reeducated Residential Directors and DSP's as to the regulations pertaining to Outside Walkways at all times. See submitted document. 12/31/2025 Implemented
6400.112(e)The record for the fire drill conducted on 11/12/2025 did not document the exit route used during the fire drill. This should actually have been cited under .112c KSA fire drill shall be held during sleeping hours at least every 6 months. Residential Director and DSP on duty at the time of the drill were questioned as to the exit used they recalled it was the front door see document submitted. Both parties were retrained in close scrutiny for no omissions on fire drills moving forward. 01/06/2026 Implemented
