6400.151(a)Direct Service Worker #1's most recent physical examination was completed on 01/16/20. This exceeds the every 2 years requirement. A staff person who comes into direct contact with the individuals or who prepares or serves food, for more than 5 days in a 6-month period, including temporary, substitute and volunteer staff, shall have a physical examination within 12 months prior to employment and every 2 years thereafter. Director Care Worker #1 went to their doctor and obtained a physical on 9/26/24. S/He was able to work with individuals with IDD again as of 9/27/24, due to obtaining her Physical Examination. The Emmaus Office Manager /Training Coordinator sent a message of reviewal to Relias' Engineer to inquire as to how this was missed. Unfortunately Relias was unable to provide more information regarding items lost from 2020-2022. The Emmaus Office Manager /Training Coordinator and HR Director will cross reference employees with the Relias training record going forward every month to ensure compliance. 10/04/2024 Implemented
6400.46(d)Direct Service Worker #1 last received training by an individual certified as a trainer by a hospital or other recognized health care organization, in first aid, Heimlich techniques and cardio-pulmonary resuscitation with an in-person skills demonstration on 06/25/20. The certified training expired 6/30/2022.Program specialists, direct service workers and drivers of and aides in vehicles shall be trained within 6 months after the day of initial employment and annually thereafter, by an individual certified as a training by a hospital or other recognized health care organization, in first aid, Heimlich techniques and cardio-pulmonary resuscitation.Direct Service Worker #1 received First aid, Heimlich techniques and cardio-pulmonary resuscitation with Emmaus's RN on 9/26/24. S/He was able to provide direct care services to individuals with IDD as of 9/27/24. Direct Service Worker #1 will be recertified in 2 years, 10/2026, in this training. The Emmaus Office Manager/ Training Coordinator requested an explanation of the Relias Training system Engineer to inquire on Direct Service Worker #1's records. 6400.73(a)During the inspection conducted 9/27/2023, there was no handrail on the three steps, in the back of the home leading to the concrete walkway. Each ramp, and interior stairway and outside steps exceeding two steps shall have a well-secured handrail. The two steps up onto a wall, which counts as 3 steps total, will have a secure handrail installed to meet requirements of regulation 6400.73(a). This location is a leased home and we gained approval from the owners to meet this regulation. The Facilities Manager is in the process of installing a new handrail that will be safe and in working order for the 3 steps. The Facilities Manager began this project on 10/12/23 and has a goal for completion early next week. No one is currently living at this location so therefore, not a hazard. But, regardless this regulation will be maintained going forward. A well-secured handrail will be in working order. 10/20/2023 Implemented
6400.80(b)During the inspection conducted 9/27/2023, the three exterior stairs in the back of the home leading to the concrete walkway, are not secured and move when stepped on. The outside of the building and the yard or grounds shall be well maintained, in good repair and free from unsafe conditions.The facility Manager began working on securing the steps in the back of the home, but only two steps were technically involved. The third step is onto a wall that is secure. The Facilities Manager began this project on 10/12/23 and has a goal to be done by early next week. No one is living at this location at the time so it does not prevent itself to be a unsafe condition at this time. However, the steps will be corrected by the Facility Manager ASAP and are in the process. 10/20/2023 Implemented
6400.101During the inspection conducted 9/27/2023, there was a turn lock and deadbolt on the door inside of the basement leading to the garage, and no exit door from the garage to the outside posing a potential entrapment risk.Stairways, halls, doorways, passageways and exits from rooms and from the building shall be unobstructed. 6400.101 No swing doors , or door with Chain locks/hotel type locks, etc. The garage is not considered an exit, as in the event of a fire, the power would be inoperable, therefore garage doors would not be able to open via an individual with IDD. Regulation 6400.101 was fixed upon Inspection completion on 9/28/23 by the Emmaus Facilities Manager. This regulation was discussed at a Management training, via the CEO, that was on 10/3/23, where the Facilities Manager and the Director of IDD Services participated in. 10/20/2023 Implemented
