On 3/12/15, WRC submitted a Reasonable Accommodation and/or Modification Request Form to the Assistant Manager of the Carriage Park apartment complex; this form was then sent to their Corporate Office for approval. WRC is requesting to attach a regulator mixer to where the hot and cold water join to prevent the water temperature from exceeding 120 F. 6400.71The telephone numbers of the police department, fire department and ambulance were not on or by the telephone in the living room.Telephone numbers of the nearest hospital, police department, fire department, ambulance and poison control center shall be on or by each telephone in the home with an outside line. A list of Emergency Phone Numbers including Police, Fire and Ambulance was present and posted near all landline telephones at the time of inspection; stickers were also secured directly to the telephone receiver and/or base, but 911 was listed as the contact number for all. New Emergency Phone Number Lists were created on March 5th, 2015 by WRC Administrative Assistant and immediately distributed to all sites by Program Site Managers; 911 is included as a contact for emergencies, but direct phone numbers to the local Police Department, Fire Department, Hospital and Ambulance Service have been provided. Direct phone numbers for Crisis Resolve Network and the Poison Control Center are also included.
