6400.163(c)Individual #1 is prescribed Seroquel for anxiety and the last reviews were done on 2/5/13, 8/6/12, 7/5/12, and 5/23/12. (c) If a medication is prescribed to treat symptoms of a diagnosed psychiatric illness, there shall be a review with documentation by a licensed physician at least every 3 months that includes the reason for prescribing the medication, the need to continue the medication and the necessary dosage. The citation was received for an individual whose neurologist prescribed an anti-anxiety medication due to symptoms of dementia and medication reviews did not occur every 90 days. This individual's last medication review was 5/28/13 and his next appointment is scheduled for 8/5/13. The house nurse will ensure that he has a medication review at least every 90 days. The root cause of this violation was that the house nurse thought only medication prescribed by a psychiatrist had to have a medication review every 90 days. Re-training has occurred with all nursing team members and Program Specialists. In addition, the Nursing Supports Managers will review the medications of all individuals quarterly to ensure that 90 day medication reviews are occurring as needed. The first review will occur by 9/30/13. 07/31/2013 Implemented
