| 65- During licensing inspection on 10/29/2025, it was found that the bathroom located in the basement of the home did not have any form of ventilation. There are no windows in the bathroom nor is there a mechanical ventilation system in place.
On 10/29/2025, TTSR Maintenance employee went to the site and installed a mechanical ventilation unit on the ceiling of the room near the bathroom door. Attached is a photo of the ventilation system (Image #1) That shows the ventilation system in place. TTSR Maintenance checked to ensure that the ventilation unit was operable before departing the site.
A review of regulation 65 took place 10/30/2025. This review included the Site Supervisor as well as the TTSR Maintenance employee. Both acknowledge the need for ventilation in living areas, recreation areas, dining areas, individual bedrooms, kitchens and bathrooms to ensure air flow by window to outside or by mechanical ventilation. Compliance will be maintained by routine site inspections and monthly walkthroughs by both Site Supervisor as well as TTSR Administration and Maintenance department to ensure that this ventilation unit continues to remain operable and if repairs are needed, TTSR Maintenance will respond promptly to repair the unit if inoperable. |

| On 10/29/2025, during annual licensing inspection, it was learned that there was no lighting system or light source on the exterior of the home to allow light to shine down on the exit. This violation was found at the residence.
Head of Maintenance for TTSR, Inc. arrived on site and brought a motion sensor lighting system was installed by TTSR Maintenance Head on 10/29/2025. TTSR Maintenance Head ensure that the motion sensor light was operable before leaving the site. Attached is a photo of the operable motion sensor light that was installed on the exterior of the home above the exit to the garage area.
A review of regulation 66 took place on 10/30/2025. Involved in the review were TTSR Maintenance Head, Site Supervisor, as well as TTSR Compliance Officer. The topic of review was regulation 66 and the need for there to always be lighting in all rooms, hallways, interior stairways, outside steps, outside doorways, porches, ramps, and fire escapes

| 6400.67(b) | On 10/29/2025, at 12:20 PM, the right side of the home's entry door frame had two protruding metal hinges for a screen door that had been removed. Both hinges had exposed, pointed ends and the bottom one, which was rusted and located near the door's threshold, posed an additional tripping hazard. At 12:29 PM, the end of the tub's spout in the full bathroom, located on the home's main level, was decomposed and rotted with hard-water deposits, oxidation, and missing material that exposed sharp, jagged metal ends. | Floors, walls, ceilings and other surfaces shall be free of hazards. | It was found during licensing inspection that the faucet on the main bathroom bathtub had a faucet that was corroded and rusted and had jagged edges. TTSR Head of Maintenance took and threw away the old faucet spicket and replaced it with a brand new spicket. Attached with POC is a picture of the new faucet spicket taken on 10/29/2025.
A review of regulation 67(b) took place on 10/30/2025. The topic of discussion was 67(b) and the responsibility of all parties to ensure that all floors, walls, ceilings, and other surfaces shall be free of hazards. All parties trained were reminded that this regulation includes all aspects of the home and non-compliance will be found if anything in the home presents a hazard or potential hazard.
Compliance will be assessed through routine monthly inspections by site staff, site supervisor, as well as TTSR Administrative staff and Maintenance (unannounced site inspections).
During licensing inspection at 5080 Cherry Run Road in Rimersburg, PA, it was found that there were two hinges/ brackets present at the top and bottom of the side of the door frame which was causing the door to protrude. There was no screen door present hence there was no need for the two brackets/ hinges to be present.
On 10/29/2025, TTSR Maintenance Head arrived at the site and removed the two brackets from the side of the door frame. Attached is a photo of the front door. The photo shows that both the upper and lower brackets/ hinges were removed and are no longer present.
A review of regulation 67(b) took place on 10/30/2025. The review consisted of reading the regulation and acknowledging that they understand the need to ensure that at all times, floors, walls, ceilings, and other surfaces shall be free of hazards. Compliance will be maintained by routine site inspections and monthly walkthroughs by both Site Supervisor as well as TTSR Administration and Maintenance department to ensure that all surfaces of free of hazards throughout the interior and exterior of the home and if repairs are needed, TTSR Maintenance will respond promptly to repair the issue in a timely manner.

| 6400.72(b) | On 10/29/25, at 12:26 PM, the bottom of the screen frame in the home's living room window located closest to the entry door was bent upward, exposing a one-half inch gap along its bottom center portion and the window frame. | Screens, windows and doors shall be in good repair. | During licensing inspection , it was found that the screen on the window located on the exterior of the residence was bent on the screen causing a one inch gap between the screen and the window frame.
TTSR Maintenance Head arrived at the site on 10/29/2025 and bent the screen back into normal position and ensured that there was no gap between the screen and the window frame. Attached is a photo of the screen that had been repaired by TTSR Maintenance Head taken on 10/29/2025.
A review of regulation 72(b) took place on 10/30/2025. The review consisted of reading the regulation and acknowledging that they understand the need for all screens, windows, and door to be in good repair at all times. Compliance will be maintained by routine site inspections and monthly walkthroughs by both Site Supervisor as well as TTSR Administration and Maintenance department to ensure that all windows, doors, and screens are in good repair and if repairs are needed, TTSR Maintenance will respond promptly to repair the issue in a timely manner.

| 6400.73(a) | On 10/29/25 the railing alongside the set of nine exterior steps located on the home's sidewalk was unsecure and loose, as the railing moved outward from the structure of the steps approximately one inch when in use. The railing's entire structure was also leaning outward from this set of steps at an approximate 120-degree angle, and its securing screws were not secured to the framing of the stairs. | Each ramp, and interior stairway and outside steps exceeding two steps shall have a well-secured handrail. | 73(a)/ 80(b)- During licensing inspection, it was found that the outside side of the house stairs that lead to garage area had was in poor repair and posed a possible danger to anyone walking on them. This stairwell's first and sixth step treads in ascending order were slanted downward and unlevel. | 6400.106 | The home's most recent furnace inspection was completed on 01/08/25. Documentation of a furnace inspection completed in 2024 was not provided; therefore, compliance with the annual requirement could not be measured. | Furnaces shall be inspected and cleaned at least annually by a professional furnace cleaning company. Written documentation of the inspection and cleaning shall be kept.
| 6400.106- During annual licensing inspection, a review of furnace inspections took place for the 2024 and 2025 calendar years. Although the 2025 furnace inspections were found and submitted for review, the 2024 furnace inspections were either not found for review or were found but did not have a date on the inspection form for that site.
Attempts were made to contact Dunkle's Plumbing, Electric, Heating, and A/C (service provider for all furnaces agency-wide) and they servicer was unable to provide proof of the furnace inspection/ service for 2024. TTSR spoke to the employee of the servicer and stated the need for all forms to be completed in their entirety to meet compliance standards. Servicer stated that they will ensure that all forms state the location and date of the inspection/ service moving forward. Once the inspections/ service is complete, Dunkle's will provide Inspection forms to TTSR. To ensure compliance moving forward, TTSR Head of Maintenance will receive these Inspection forms and will review for completeness. If it is found that there is insufficient or missing information on any of the individual site Inspection forms, TTSR Head of Maintenance will follow up with service provider and will request additional information on each form to meet compliance standards. A review of regulation 6400.106 took place on 11/10/2025. Head of maintenance was reminded of the importance of reviewing these documents to ensure that all sites have operable and safe units in the home and that there is sufficient documentation to support that the units are operable and safe in each home.
